What to Bring
At your first visit please bring the following information:
- Completed Health History Form
- Insurance Information
- Signed acknowledgement of Private Policy Practice
- A copy of any Panoramic or Full Mouth X-rays taken within the last year (if available)
The Orthodontic Process
The patient and his/her family are greeted and welcomed as they enter the office. They are shown how to sign in for subsequent appointments, and they are given a health history form and notice of privacy practices to fill out if they do not already have them.
Dr. Broccoli will introduce himself to you and your family and proceed with the initial exam. The patient’s health history will be discussed, and the reasons for seeking treatment will be explored. Dr. Broccoli will perform a thorough orthodontic exam and will make recommendations as to the need and/or timing of treatment. During this initial exam, Dr. Broccoli will take all of the time needed to answer all of your questions.
If treatment is indicated and you would like to proceed, we will schedule an appointment for diagnostic records. The diagnostic records appointment takes about 30 minutes and involves x-rays, digital photos of the patient’s face and teeth, and a scan of the teeth to create a digital 3d models.
The information gathered at this appointment is studied and used to develop a customized treatment plan for each patient.
